• The rate it uses.
• The business trips grouped by client or project, with each one’s date, where and why, miles and amount.
• The totals, what was already reimbursed, and the balance due.
• Lines to sign and date.

With a plug-in car charged at home, it can add the home charging in the period (kWh and what it cost) for an employer who pays for it.
